We’re seeking a results-driven Community Engagement Manager to expand our reach, build strong referral networks, and generate new client leads through strategic community-based marketing initiatives.

 

Position Summary:

The Community Engagement Manager is responsible for driving growth by increasing awareness of BCHC’s services through targeted outreach and marketing. This role will focus on building and nurturing high-value relationships with referral partners (discharge planners, nurses, social workers, physicians, and other medical professionals) to capture referrals and strengthen our presence in the community.

Key Responsibilities:
  • Develop and execute marketing and outreach strategies to increase referrals and client acquisition.
  • Identify, establish, and maintain strong relationships with key referral sources, including hospital discharge planners, nurses, social workers, physicians, and senior living communities.
  • Conduct regular visits to hospitals, clinics, and community organizations to promote BCHC’s services and educate staff on referral processes.
  • Plan, coordinate, and attend community events, health fairs, and networking opportunities to increase brand recognition and lead generation.
  • Collaborate with internal teams to develop marketing campaigns, promotional materials, and digital content tailored to referral partners and community audiences.
  • Track, analyze, and report on outreach activities, referral generation, and lead conversion metrics.
  • Act as a brand ambassador and subject matter expert in the community, ensuring consistent messaging and positive representation of Bloom Concierge Home Care.
  • Organize educational presentations and in-service trainings for discharge teams, social workers, and other healthcare professionals to position BCHC as the preferred home health care provider.

 

Performance Metrics:

Success in this role will be measured by:

  • Referral Growth: Achieving monthly and quarterly targets for new client referrals from hospitals, physicians, social workers, and discharge teams.
  • Relationship Building: Establishing and maintaining a minimum number of new referral source contacts (e.g., X new contacts per month) and maintaining regular follow-ups.
  • Event Participation: Hosting or participating in a set number of community or professional events per quarter to build brand awareness.
  • Lead Conversion: Tracking referral-to-admission conversion rates and improving conversion through relationship management and follow-up.
  • Market Expansion: Increasing the number of active referral sources and identifying new untapped referral channels.

 

Qualifications:
  • 2+ years of experience in marketing, business development, or community outreach (healthcare or home health care experience strongly preferred).
  • Proven track record of building referral networks and increasing client leads.
  • Strong communication, networking, and presentation skills.
  • Highly organized with excellent project management and follow-through abilities.
  • Self-starter who thrives in both independent and team environments.
  • Knowledge of local healthcare systems and community resources is a plus.
