Position Requirements:1. High School Diploma2. Valid Drivers License3. Successful completion of training requirements (HFS 88 and other company training within three months of appointment to this position.)4. Must have effective oral and written communication skills.5. History of working with persons with a developmental disability, mental illness, elderly, and traumatic brain injury6. Must be able to use Microsoft Office, computers, and various office equipment.7. Must have open availability and available on call and willing to work all location Available to fill in as PCA. The Essential Job Functions Include: Supervision of Residential Care Staff- in the daily management of Program/Residents Participate in Interviewing, hiring, training, coaching, discipline, evaluations and assisting with termination of Residential Care Staff. Schedule, coordinate and provided planned care to the clients of the facility in a clean and safe living environment as required by HFS 88. Develop and implement an Individualized Service Plan (ISP) and BSP for each client of the program/facility as required by HFS 88. Oversee all resident documentation- State Regulatory compliance and company Standards. Communication of all facility maintenance needs and ensure compliance for fire safety, medication administration, first aid and choking, standard precaution, patient rights and health monitoring requirements. Update or initiate assessment/risk analysis of current potential clients for consideration of placement at the program/facility. Coordinate the arrangement of transport or transport directly clients to scheduled appointments and per individual need/request. Provide required daily, weekly and monthly progress reports, critical incident reports and Individualized Service Plan (ISP) updates to the client case manager, guardian and others. Assists in the administration of medications that are ordinarily self- administered. Reports any observed or reported changes in the clients condition and / or needs. Ensure Documentation of cares provided and complete the forms required for the clients records. Complete the appropriate records to document cares given and pertinent observations. Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines and reporting unsafe situations to the director. Demonstrates safe practice in the use of equipment. Notifies director of educational needs. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested. Maintains confidentiality in all aspects of the job Participates in the development, implementation, and evaluation of the Agency Quality Improvement Program and pertinent activities. Participates in the development, implementation of enrichment activities for program members. Performs other related duties and responsibilities as deemed necessary.
